2. To put up a claim, simply download the Claim Form for submission.


3. Return the duly completed Claim Form with supporting documents, e.g. air tickets, boarding pass, medical certificate or receipt, police report, letter from airline, copy of passport showing duration of trip (for annual plan), etc.
